How does treatment with chemical peel work?

A chemical peel is a skin treatment in which a controlled acid solution is applied to the skin to remove dead skin cells and stimulate skin renewal. Different acids at different strengths are used depending on the goal: superficial peels (AHA, BHA) give fresh skin with no downtime, medium-depth peels (TCA, Jessner) treat pigmentation and fine lines, and deep peels (phenol — rare in Sweden) are used for severe signs of ageing or scars. Chemical peels are performed by a registered nurse or a trained medical skin therapist, and a skin analysis is mandatory before the first treatment in order to choose the right strength for your skin type.

How the treatment is carried out — step by step

  1. 1

    Consultation and skin analysis

The practitioner assesses your skin type (Fitzpatrick I–VI) and goals (acne, pigmentation, fine lines, texture) and recommends a type and strength of peel. For medium-depth peels, home preparation for 2–4 weeks with specific skincare (vitamin A, acids) is often recommended to maximise the result.

  2. 2

    Cleansing and degreasing

The skin is washed thoroughly and degreased with a special solution so that the acid can penetrate evenly. Make-up and skincare residue must be completely removed.

  3. 3

    Applying the acid

The acid solution is brushed onto the skin in one or more layers. You may feel a stinging, burning sensation that increases with the number of layers. The practitioner monitors the reaction and stops exactly when the right depth has been reached (with medium-depth peels, "frosting" — a white precipitate on the skin — is seen).

  4. 4

    Neutralisation and soothing

Some peels require neutralisation with an alkaline solution (Jessner, glycolic acid); others are absorbed on their own (TCA). A soothing mask, a cold compress and a thick moisturiser are applied. Sun protection completes the treatment.

  5. 5

    Treatment series and maintenance

Superficial peels: 4–6 treatments every other week, then monthly maintenance. Medium-depth: 1–3 treatments 4–6 weeks apart, then maintenance every six months. For severe pigmentation or acne, a combination with microneedling or home care may be recommended.

Recovery and aftercare

Recovery time varies greatly with peel depth. Superficial peel (AHA, glycolic): no visible downtime. Medium-depth (TCA, Jessner): 5–7 days of peeling, during which the skin flakes off in layers. Deep peel: 1–2 weeks at home with strict aftercare. Plan important social occasions for after the recovery period.

  • Use SPF 50+ daily for at least 4 weeks after the treatment — sun protection is CRITICAL to avoid hyperpigmentation.
  • Do not pick, scratch or pull at peeling skin — this can cause permanent scarring or pigment changes.
  • Use only the skincare the clinic has recommended (usually a gentle cleanser + a thick moisturiser with healing ingredients such as panthenol).
  • Avoid active ingredients (retinol, AHA, BHA, vitamin C) for 1–4 weeks depending on peel depth.
  • No exercise, sauna, swimming pool, hot baths or sunbeds for the first 7 days after a medium-depth peel.
  • Sleep with your head raised for the first few nights to reduce swelling.
  • Drink plenty of water — it supports the skin's healing.
  • Avoid sex, alcohol and spicy food for the first 24 hours after a medium-depth peel — they increase swelling.
  • No other aesthetic treatment in the area within 4 weeks.
  • In case of blisters, yellowish fluid or a prolonged burning sensation — contact the clinic.

Who is not suited to chemical peel?

The treatment is not suitable in the following situations. Always mention these during the consultation.

  • Pregnancy and breastfeeding (applies in particular to salicylic acid/BHA and certain retinol-based peels).
  • Active infection in the treatment area (bacterial, viral, fungal).
  • Active herpes infection, or a known susceptibility to herpes without prophylactic antiviral treatment.
  • An active acne breakout with inflammation (for deeper peels) — can worsen the infection.
  • Eczema, psoriasis or rosacea in an active flare.
  • Ongoing isotretinoin treatment (Roaccutan) or treatment stopped within the last 6 months.
  • Sunburnt skin or skin covered in self-tan — wait until it has evened out.
  • Tendency to keloid or hypertrophic scarring — risk of worsened scarring with deeper peels.
  • Dark skin (Fitzpatrick V–VI) requires an experienced practitioner and often lower strengths — increased risk of pigment changes.
  • Previous allergic reaction to any ingredient in the peel solution.
  • Weakened immune system or ongoing chemotherapy.

Differences between brands

Several different products are used in Sweden. The differences are often small — which brand suits you is decided by your practitioner based on your goals and previous treatments.

AHA / Glycolic acid (superficial)

The mildest class — alpha hydroxy acids such as glycolic acid and lactic acid (10–70% strength). Good for freshness, radiance and mild exfoliation. No downtime; suitable as a monthly "refresher" treatment.

BHA / Salicylic acid (for oily skin)

A beta hydroxy acid that dissolves in oil and penetrates into the pores. Good for active acne, blackheads and oily skin. Mild peeling for 2–3 days.

Jessner peel (medium-depth)

A classic blend of salicylic acid, lactic acid and resorcinol. Gives controlled peeling over 5–7 days. Effective against pigmentation spots, fine lines and oily skin.

TCA peel (medium-depth to deep)

Trichloroacetic acid at 15–35% strength. A stronger effect on pigmentation, fine lines and acne scars. 5–10 days of visible peeling. Only for trained practitioners.

PRX-T33® (revolutionary hybrid)

An Italian peel that combines TCA + hydrogen peroxide + kojic acid with no downtime. Stimulates deeper skin layers without surface peeling — popular among people who cannot take downtime.

Cosmelan® / Dermamelan® (pigmentation peel)

Specialised peel protocols for melasma and severe hyperpigmentation. The mask is applied at the clinic and worn for 8–12 hours, followed by home care for 6 months. Very effective against pigment.

When will you see the results?

  1. Immediately afterThe skin may be red, warm or "frosted" depending on peel depth. Slight stinging is normal with medium-depth peels.
  2. Day 1–3With medium-depth peels: the skin darkens and starts to peel. With superficial peels: mild redness fades. No social restrictions with superficial peels.
  3. Day 3–7Medium-depth peels: active peeling, with the skin flaking off in layers. Picking = scars. Do not scratch!
  4. Week 2New, brighter and smoother skin emerges. Radiance is visible.
  5. Week 4–6Time for the next peel in a series. Skin quality has improved cumulatively.
  6. Months 3–6Long-term collagen production brings continued improvement in texture, fine lines and pigmentation.

Risks and what to consider

Common side effects
