How does treatment with botox work?

Botulinum toxin type A — in Sweden most commonly known by the brand name Botox® — is an injection that temporarily relaxes selected facial muscles. As a result, wrinkles caused by facial expressions (forehead lines, frown lines between the eyebrows and crow's feet) become less prominent. Botulinum toxin is a prescription medicine and in Sweden may only be injected by a licensed physician, dentist or nurse after a consultation and the statutory cooling-off period.

How the treatment is carried out — step by step

    Consultation and assessment

    The practitioner goes through your medical history, medications, any allergies and what you want to achieve. Your facial expressions are analysed to assess which muscles need to be treated. Before an aesthetic injection treatment, a cooling-off period of at least two days applies after you have received the information about the treatment. If the same type of injection treatment is repeated within six months, no new cooling-off period is required.

    Skin preparation

    The skin is cleansed and disinfected. Anaesthesia is rarely needed because the needle used is very fine, but a cold pack or local anaesthetic cream may be offered for sensitive areas.

    The injections

    The practitioner marks the points and injects small doses of botulinum toxin into selected muscles. The number of injections depends on the treatment area — often 3–5 points per area. Most people feel only a slight prick.

    Immediately after the treatment

    You can leave straight after the treatment. Slight redness or small needle marks may be visible for a few hours. You receive written information about what to avoid during the first day.

    Follow-up

    The effect is evaluated at a follow-up visit after 2–3 weeks. The dose is adjusted if needed — it is common to need a minor adjustment at the first treatment to find the right level for you.

Recovery and aftercare

Botox requires no real recovery — you can go back to work the same day. However, there are a few things you should avoid for the first 24 hours so that the medicine settles correctly in the treated muscles.

  • Avoid massaging, rubbing or pressing on the treatment area for 24 hours.
  • Avoid intense exercise, saunas, sunbeds and hot baths on the same day.
  • Sleep with your head slightly raised the first night.
  • Avoid blood-thinning medicines (e.g. ibuprofen, aspirin) for 24 hours if medically possible.
  • Do not use face masks or strong skincare (peels, retinoids) during the first day.
  • Contact the clinic if you experience unusual weakness, drooping eyelids or difficulty swallowing.

Who is not suited to botox?

The treatment is not suitable in the following situations. Always mention these during the consultation.

  • Pregnancy or breastfeeding.
  • Known hypersensitivity to botulinum toxin or to any of the medicine's excipients.
  • Active infection or skin disease on the treatment area.
  • Neuromuscular diseases such as myasthenia gravis, Lambert-Eaton syndrome or ALS.
  • Ongoing treatment with certain antibiotics (e.g. aminoglycosides) that can enhance the effect.
  • Bleeding disorders or ongoing treatment with blood-thinning medicines — discuss with your doctor first.
  • Unrealistic expectations — botox cannot remove deep, static wrinkles that are present at rest.

Differences between brands

Several different products are used in Sweden. The differences are often small — which brand suits you is decided by your practitioner based on your goals and previous treatments.

Botox® (Allergan/AbbVie)

The best-known and longest-studied brand on the Swedish market. Active substance: onabotulinumtoxinA. The effect typically appears within 3–7 days.

Azzalure® (Galderma)

Active substance: abobotulinumtoxinA. Dosed in different units from Botox and, according to some studies, has a slightly faster onset of effect.

Bocouture® / Xeomin® (Merz)

Active substance: incobotulinumtoxinA. Lacks accessory proteins, which theoretically reduces the risk of the body forming antibodies with repeated treatment.

Alluzience® (Galderma)

Ready-mixed liquid form of abobotulinumtoxinA — reduces the variation that arises when it is mixed at the clinic.

When will you see the results?

  1. Day 0 (after treatment)No visible changes — you may see small needle marks or slight redness.
  2. Day 3–7The effect starts to set in. Facial expression around the treatment area feels gradually more relaxed.
  3. Week 2–3Full effect is reached. This is when the practitioner evaluates the result at a follow-up visit.
  4. Months 3–4The effect slowly starts to wear off — muscle activity gradually returns.
  5. Months 4–6Time for a new treatment if you want to maintain the result. With repeated treatments, the effect may last somewhat longer.
